Configure Lead Forms

Define the answers CJM recognizes, qualify leads, and choose how to receive submissions.

Lead Forms lets you configure how CJM reads a form already on your website, in Meta Lead Ads, or in HubSpot. The configuration defines questions, which answers count toward qualification, and how a submission connects to a profile.

It does not automatically create a form page on your website. For a custom form, send submissions through the Pixel or a webhook; Meta and HubSpot send submissions from their imported forms through their integration.

Configure and publish

  1. Open Setup → Lead Forms.
  2. Create or import the form from its source card.
  3. Open the form and add or edit questions in the Draft version.
  4. Set the category and answer level for answers you want to use to qualify a lead.
  5. Select Save Draft, check the preview, then select Publish Draft.

The Live version remains in use until you publish the draft. This lets you prepare changes without changing the form already in use.

Review captured submissions

Open a form, then select the Submissions tab to see every submission CJM captured for it. Select a row to open its detail and review the source, submission time, contact details, and recorded answers.

If a test submission is missing, first check that you used the published form configuration and the correct webhook or Pixel identifiers. A draft by itself does not change the Live form that receives submissions.

Check before publishing

  • The form includes the fields needed to identify the person submitting it.
  • Every choice question used for scoring has configured options.
  • For a website form, the form and question identifiers match what the Pixel or webhook sends.
